Humanitarian Pacemaker Implantation in Lima's
Hospital Dos de Mayo Since 2012
Every year, thousands of Peruvians die from remediable cardiovascular ailments because they cannot afford or access even the most basic medical care. Lifebeat Peru's mission since 2012 is to make as big of a difference as possible by providing cardiac care. With over 10 years of pacemaker donations from Medtronic and Boston Scientific, hundreds of Peruvians once facing terminal heart conditions can now live healthy lives without any financial burden.
Each year, Hospital Dos de Mayo compiles a list of the patients that most urgently require pacemakers or defibrillators and are also least able to afford this treatment. The usual pacemaker recipient comes from Barrios Altos in Lima and typically plays a crucial role in supporting their own family, a crucial factor in why they need emergency intervention. However, patients also come from all over Peru---in one case, a recipient bravely traveled 3 days by boat, bus, and on foot to arrive at Dos de Mayo.
The average Lifebeat pacemaker recipient typically suffers from heart block, a condition in which electrical cardiac signals are partially to completely lost through decreased function of the atrioventricular node. As a result, the average Lifebeat's heart rate is typically in the 20-30 BPM range. This is not life sustaining, and as a result, they require the immediate implantation of a life-saving pacemaker or defibrillator. However, patients also suffer from a wide array of arrhythmias and related comorbidities that require medical attention.
Built in 1875, Hospital Dos de Mayo is Lima's first and largest public hospital, providing crucial services for a large part of the community in the Barrios Altos district of Lima. Besides generous donations from the South Korean government, however, the hospital's layout has remained largely unchanged.
This photo of Dr. Albrecht in 2019 reflects the reality of Dos de Mayo. Aside from operating rooms, most patients are grouped together in 8 open air wards surrounding the central courtyard shown above. Each general ward typically has one or two computers---the vast majority of work in Dos de Mayo is still done on paper.
Although Lifebeat implants pacemakers in Peruvians from all over the country, the majority of recipients come from the district where Hospital Dos de Mayo is located, Barrios Altos. As seen in the photo, the population of Barrios Altos is largely comprised of underserved neighborhoods with no direct access to plumbing services or potable water.
The operating room where Lifebeat has performed all pacemaker implantations since 2012. The Philips and Siemens equipment has been used several times daily for nearly 15 years and with little access to proper maintenance.
2023 was Lifebeat's biggest year yet in its 12 years of operation, with an incredible 14 Boston Scientific pacemakers, 12 Medtronic pacemakers, and 4 defibrillators! Consequently, we had a tight schedule for the week of implantations in Dos de Mayo. On the second day, the blessing of so many donations became a challenge when the operating room Lifebeat has used for a decade broke down (see images above). However, thanks to the determination of the team and those working in Dos de Mayo, other operating rooms were secured for implantation and all devices were implanted on schedule!
As always, our trip would not have been possible with the tenacity, resourcefulness, and caring displayed by all the staff in Hospital Dos de Mayo. Despite major changes to where and how the pacemaker operations were performed for the first time in 12 years, the workers involved did everything to succeed implanting the pacemakers in every patient on the donation list.
